Jupiter Family Healthcare Policies

We strive to provide the best care to our patients in an efficient manner. There are a few policies we have to help provide this care:

  • Please notify the staff at Jupiter Family Healthcare of any changes in insurance coverage or address changes.

  • Please bring your insurance card and driver’s license / ID to every appointment.

  • In accordance with insurance contracts, co-payments and/or deductibles are due at the time the services are rendered.

  • Please bring all medication bottles to each appointment so we may verify correct dosing.

  • Please provide 24 hours notice for any appointment changes or cancellations so that we may provide this appointment to another patient. This will avoid any No Show fee that would occur.

  • Please try to obtain all medication refills at your office visits. Only patients, not pharmacies, may call in for refills to minimize errors. Allow 24 to 48 hours for prescription refills.

  • Due to charges from our bank, there will be a $35 to $50 charge on returned checks in addition to the amount of the check.

  • Patients will be responsible for any balance not covered by the insurance carrier or Medicare. Any patient balance greater than ninety (90) days old will be sent to collections. An interest rate of 1.5% per month will be added to all delinquent accounts.

  • For hospital admissions, patients will be seen by our designated hospitalist. We have found this to be better for patient care since the hospitalist is available at the hospital 24 hours a day and then we are able to provide extended care in the office. The hospitalist is in contact with your Jupiter Family Healthcare provider at all times.
